  • If your child recently received a notice concerning immunization requirements, please see the following:

In addition to your pediatric clinic, the Arkansas Department of Health and many local pharmacies will provide immunizations. Please arrange for your child to receive all required vaccines and communicate with the school nurse by September 15 that your child is now up to date, or that you have scheduled an appointment and are working to resolve this issue.

To apply for an immunization exemption, complete the following application from the Arkansas Department of Health, and provide proof of exemption to Jessieville School District.

    Exemption Application

Students who are not current on immunizations by October 1 and have not submitted proof of exemption are at risk of not being permitted to return to campus. For more information about your child’s immunization record, please contact the school nurse.
